Health and Housing Coordinator

Salary: £12.50 - £15.50 p/h

Hours: 37.5 per week.

A fantastic opportunity for an experienced full-time Health and Housing Coordinator role based at Margate, to step down bed facilities and with an element of home visiting incorporated.

To work both as an integrated member of the Rapid Transfer service team and also to undertake follow up from home visits. You will aid a robust approach to support patient discharge, quickly, safely and effectively.

This role is seen as an integral part of the Rapid Transfer Service, responsible for avoidance of hospital admissions whenever possible along with providing a timely proactive discharge planning service to ensure.

  • Support planning with best possible outcomes for the patient.
  • Timely access to range of community based health, social care services and voluntary sector services.
  • Optimum is of acute/community and social services resources.

Essential criteria:

  • Experience of providing support to the designated client group either in a volunteering or professional role
  • Must have a full UK driving licence and use if a car (business insurance).
